We thank Drs. Kaufmann and Wilkinson (Kaufmann and Wilkinson, 2016) for commenting on our paper (Kameyama et al., 2016), but we are disappointed that they have entirely misinterpreted the intent of our study. Lateral spread, which is responsible for an abnormal muscle response (AMR), takes place at or near the compression site and not, as they propose, at the facial nucleus, but our study does not exclude central hyperexcitability, which we believe is also relevant, based on many previous reports including our own (Kimura et al., 1975).
