Cystic lesion of the ventriculus terminalis accompanied by split cord malformation

The ventriculus terminalis (VT) is a cavity situated at the level of the conus medullaris (CM). It is enclosed by ependymal tissue and almost exclusively in pediatric age (1, 2). In pathological conditions, a cyst of the CM is probably the result of a persistent VT (3, 4). A 45-year-old man with a cyst of the conus presented with bilateral lower extremity pain and left side dominant tingling and burning. Symptoms were present for several years but worsened in the last year. MR images revealed dermal sinus tract and bifid spine in the sacral first vertebra level (Fig 1) and a cystic lesion of the CM that is accompanied by split cord malformation (Fig 2).
