Volumetric laser endomicroscopy in the detection of neoplastic lesions of the esophagus

Volumetric laser endomicroscopy (VLE) in a 64-year-old man with Barrett's esophagus revealed two regions with intramucosal adenocarcinoma missed by high magnification narrow-band imaging. On VLE, these regions where characterized by lack of regular striations in the architecture, jagged shaped ducts suggestive of glandular atypia, and heterogenous signal intensity consistent with irregular nuclei (Arrows, Fig. 1; Normal, Fig. 2). Histopathology on targeted endoscopic mucosal resection showed high-grade dysplasia with focal intramucosal adenocarcinoma, and high-grade dysplasia with papillary intramucosal adenocarcinoma, respectively.

Evidence for a Complex Relationship Among Weight Retention, Cortisol and Breastfeeding in Postpartum Women

Abstract

Objective To assess the relationship between cortisol slope, a biologic marker of stress, and postpartum weight retention. Methods We included 696 women in a secondary analysis from a multi-site study conducted using principles of community-based participatory research to study multi-level sources of stress on pregnancy outcomes. As a stress marker, we included salivary cortisol slope; the rate of cortisol decline across the day. Pre-pregnancy weight and demographic data were obtained from the medical records. At 6 months postpartum, patients were weighed and returned saliva samples. We built stepwise regression models to assess the effect of demographic variables, cortisol slope and cortisol covariates (wake time, tobacco use and breastfeeding) on postpartum weight retention. Results 45.5 % of participants were African American, 29.2 % White, and 25.3 % Hispanic. Of the Hispanic women 62.5 % were Spanish speaking and 37.5 % were English speaking. In general, participants were young, multiparous, and overweight. Postpartum, almost half (47.6 %) of women studied retained >10 lbs. In multivariable analysis including age, pre-pregnancy BMI and public insurance, cortisol slope was significantly associated with weight retention (β = −1.90, 95 % CI = 0.22–3.58). However, when the model was adjusted for the cortisol covariates, breastfeeding (β = −0.63, 95 % CI = −1.01 to −0.24) and public insurance (β = 0.62, 95 % CI = 0.20–1.04) were the two strongest correlates of weight retention. Conclusions for Practice The association between cortisol slope and postpartum weight retention appears to be influenced breastfeeding status.

Topography and indexing of nutrient foramina of tibia—a study in Vindhya region

Background: Clinically, longitudinal stress fractures are more commonly associated with the tibia. The healing of fractures, as of all injuries, is reliant upon blood supply. So, detailed anatomical study of position and number of nutrient foramina is must. Nutrient foramen is an opening into the bone shaft which gives way to the blood vessels of the medullary cavity of a bone, for its nourishment and growth. Objective: To investigate the number, position, size, and direction of nutrient foramina of tibia and calculation of foraminal index (FI). Materials and Methods: The study was done in Department of Anatomy, Shyam Shah Medical College, Rewa, Madhya Pradesh, India, in 80 adult human tibiae. We have measured all the four parameters in each bone according to standard method. Result: According to FI, most of the nutrient foramina of tibia were situated in the proximal third (type 1) below the soleal line. The mean FI was 32.96 ± 6.06, with the FI ranging between 27.20% and 37.25% of the bone length. Conclusion: To conclude that our detailed study of topographic anatomy and morphology of nutrient foramina of tibia provides additional information to orthopedicians before planning surgeries on tibia.

What's the weirdest 911 call you've ever received?

A question posted recently on Quora asked, "What was the weirdest call ever received on 911"" Farah Rogers, a 911 dispatcher and trainer, gave her opinion on the topic below. Check it out and add your own thoughts in the comments.

There is no way to know what the weirdest call at any agency is but I can tell you the weirdest call I ever recieved.

I got a 911 call from a female for her husband having chest pains. The ambulance checked out on scene and it ended up being a schizophrenic male chasing a female (and now my ambulance crew) with a machete. He then took off into the woods naked with said machete. Officers got dispatched and couldn't find him, a helicopter got sent and couldn't find him.

I got a second 911 call from a cell phone pinging next door about an hour later from a man laying on a garage floor saying a demon attacked him and he couldn't get up.

I thought maybe the first naked guy had attacked him.

That caller ended up being the naked guy with the machete. He hit himself with the machete. Officers got there and he started stabbing himself in the chest. They tazed him, disarmed him, took him to the hospital and got him on meds.

He barely had cuts (maybe a dull blade). He's fine now. He brought us cookies last weekend.

Modeling the Influence of Early Skin-to-Skin Contact on Exclusive Breastfeeding in a Sample of Hispanic Immigrant Women

Abstract

Using data from a longitudinal study of breastfeeding in Hispanics, this study evaluated the influence of early skin-to-skin contact (SSC) on initiation and sustained exclusive breastfeeding (EBF) at 1 month postpartum. Two-thirds of the women in the sample participated in early SSC. At discharge, over half of the women were EBF; this proportion decreased to one-third at 1 month postpartum. Controlling for demographic and clinical variables in the model, participation in early SSC was associated with a greater than sevenfold increase in the odds of EBF at discharge (p = .005) but was not predictive of EBF at 1 month post-discharge (p = .7). Younger maternal age and increased prenatal infant feeding intention were associated with an increased likelihood of EBF across both timepoints. Promoting early SSC may help with initiation of EBF, while further breastfeeding support may be needed to maintain EBF following discharge for this vulnerable population.
